Overview
Forums comprised of the entire medical, surgical, and multidisciplinary team whereby individual complex patient cases are reviewed to determine their ability to undergo transplant evaluation, receive approval for listing to receive a transplant, and post-operative care is a need to ensure a multidisciplinary approach to the care of each transplant candidate and recipient and living kidney donor candidate and recipient. These forums also act as a didactic session for evidence-based transplant medicine in caring for the transplant population. This forum is also a requirement set forth by CMS and UNOS, which are the governing bodies for organ transplant programs. Attendees will leave the conference setting with the knowledge of the plan of care across the continuum and a knowledge base of the overall care of the patients within the program setting based on evidence based practice discussions, current research, prior patient care scenarios, and lessons learned. Formal multidisciplinary care planning in all phases of care (patient selection, pre transplant, transplant event, post-transplant) must take place with written documentation that these practices occurred. Standard of care nationwide within transplantation is in the format of a “Selection Committee” or “Multidisciplinary Review Board”, both of which follow the standard practice of gathering a group of required individuals as outlined in tag X125 to discuss patients in all phases of care – thus allowing for the introduction of knowledge, experience, patient outcome/decision making, current literature, and best practices to be reviewed as it pertains to the patients within the program. Competence - The Selection Committee activity has a direct correlation to both the attendees’ competence and performance in that – the discussions are based on current practice, best practice and how they relate to the patient at hand. Discussions at the meetings are held by people with varying experiences, degrees, and roles within transplant – all of which direct how a patient is treated - i.e., plan of care, changes in care plan, current literature in the plan of care, drugs utilized; the list goes on and on. Through the discussion surrounding varying plans of care, competence in the care of patients is built upon through the experience itself and has a direct relationship to the performance by each individual in these meetings. The plan of care has a direct relationship to the patient outcomes and in fact, can directly alter the course of the patient outcomes by action items discussed within the meetings. Performance – Items discussed within the Selection Committee as outlined above build upon competencies through discussion, experience, current practices, best practice, and research. Utilizing these items – plans of care are implemented through the individuals performing the care and even further, policies and protocols are impacted/influenced.
