Overview
he management of arteriovenous malformations (AVMs) and other cerebrovascular lesions is challenging. The rupture rate of an AVM is approximately 3% (2). The goal of treatment is to prevent a hemorrhage. This is achieved by getting obliterationof the nidus. The treatment for these lesions involves neurosurgery, radiation oncology as well as endovascular surgeons. (2) Treatment options include microsurgery, embolization and radiosurgery as well as multimodal approaches (2). Decisions on whether to treat with surgery , embolization or radiosurgery have to be done in an interdisciplinary setting. As AVM obliteration rates have improved in recent years with increasing volumes and radio surgery, there is an increasing role for radiation oncology in the treatment of these lesions and less of a role for open surgery as the risks of open surgery are large. The outcomes of radiation treatment for very large AVMs have been improved by changing the amount of radiation by prescribing an AVM margin dose of >17 Gy and adding additional isocenters to that >63% of an internal AVM does receives more than 20Gy (1). However, outcomes with radiation are variable and the unique characteristics of the AVM and radiation treatment parameters make it important to make these treatment decisions as a group (3). As the natural history of AVMs is poorly understood and management is still controversial, multidisciplinary management is crucial to good outcomes (4, 5).

Objectives
  1. Develop a comprehensive treatment plan for complex vascular cases
  2. Identify areas of institutional expertise in managing vascular lesions
  3. Review outcomes of completed complex vascular cases to determine if treatment plan was optimal
